Death notification form for the bereaved

SAIF has been asked to share these details from Suicide Bereavement UK:

“I am mindful of the thousands of people who have been recently been bereaved, during these unprecedented times.

Notifying agencies of a family member’s death is an extremely difficult, emotionally charged and labour intensive task for the bereaved.  Many are uncertain who to contact and where to start.

In an attempt to address this unmet need, My colleagues and I have developed a death notification form for the bereaved, which provides a list of organisations that they many need to (or wish to) notify.  It has been deliberately designed so that it can be used/offered to anyone who is bereaved, regardless of the cause of death.”
